- Photo of the child
- Photo of Parents
- Attested copy of Address Proof (Ration Card/ Electricity Bill/ Aadhar Card/ Agreement).
- Income certificate and Category certificate (if applicable)
- Fees via Cheque or online payment
- Attested copy of Birth Certificate
